I have a light that is fitted with a small bayonet bulb holder (SBC 15). The bulb is 12v 5watt. If I put a 240v 15watt bulb in and rewire with appropriate cable & earthing, what are the implications?

Or is a transformer the better option?

Why don't you use a 12v bulb? I would stick to that but use a higher-wattage assuming whatever you are using will allow a higher watt bulb as it would burn hotter and brighter.

I would stick with the correct voltage. there are many things that can be involved in the type of bulb specified including insulation, type of switch etc, not just an earth.

Heat may also be an issue, most light roses are limited to XX wattage, thats becasue theyre capable of dealing with XX amount of heat, same may apply to your fitting.
