as per title, how are they made? In particular how do they make the surface so neat?

Is the carbon wrapped round some former then put into a 2 part tube, vacuum formed? then polished?

... nad once that has been answered, what about carbon fibre bars?

Been asking myself the same question. From this website: http://hardyadvancedcomposites.org/carbonfibretubes.aspx it looks as if the carbon fibre is wrapped around a mandrel, which is removed from inside after the resin has been added and cured. On the machining page you can find the explanation that the wrapping tape leaves the outside of the tube textured so this can be machined off. I guess a protective lacquer is then applied.

Look at this: http://hardyadvancedcomposites.org/carbon_fibre_tube_wrapping_technology.aspx

I also found some stuff about complex shapes being internally moulded, forced into the mould by an expanding bladder.
