Stretched gear cable?

Over the past few days my 3-speed hub gears have been slipping out of gear occasionally - apparently at random, but never for more than a few strokes at a time, always in middle gear. Tightening the cable tension with the wee screw at the back fixes this temporarily (well it seems to, anyway) but after a day or two the problem returns.

Is it likely that my gear cable has stretched? Or could there be something more fundamentally wrong with the gears?
