My Acera on my Marin Larkspur is smoother than my 105 on the road bike.

I've played around with the adjustments for hours, I just cannot get it to shift though all 20 smoothly.

Looks like a trip to the LBS.

8-10 and 18-20 are really clunky.

There must be a secret formula for cable tension and rear mech adjustment. The H screw doesn't seem to get the shifts tight enough.

Could be a few thiungs, sticky cables/bent rear mech though most likely adjustment. you need to set up your derallieur

see http://www.parktool.com/repair/readhowto.asp?id=64

sounds like you might need to adjust the barrel though the h screw might need readjusting now - see the guide re setting up.

good luck

'H' is a limit adjust screw which sets the resting position of the rear mech, it doesn't 'adjust the gears' but is the starting point for gear set-up.

Remove the gear cable from the mech.
Using the 'H' limit adjust screw set the mech so that the top jockey wheel is directly underneath the smallest sprocket. (Whilst you are back there check that the mech is absolutely vertical and parallel to the sprockets.)
Screw in the barrel adjuster all the way and then undo it one turn.
Pull the cable taught and reconnect the cable.

Aside from a turn or two of the barrel adjuster that should be it.
