elbow wrist pains

I am on my forth week of touring and I've already cycled 1800km.
My legs and back are ok, but I sometimes I feel pain on my elbows and wrists. I'm ridding about 5 hours a day on a hybrid Ridgback flight T2 (maybe not the best for touring) and the bars are a little lower than the saddle which I guess means some of my weight goes through my arms. Unfortunately I can't raise the bars, neither I want to lower the saddle as it feels to be at the right height.
Is there anything I can do about it? Do you think this may cause more serious problems later on?
