SPD's don't normally release unexpecedly, but they can........happened to me on new XT pedals on a fixed....... one legged bobbing at 25 mph.........
Sometimes the cleat can wear badly - that's what happened to me, earlier and the subsequent cleats have been fine.... only had the 'bad' cleat a few months..... old cleats lasted years, fitted new, one went bad, replaced and have been fine since......
Same with any system - eliminate what is causing the issue.
I had a stack of issues with cleats at the Velodrome in November..... no problem on my pedals at home (top of the range compared to the basic pedals at the drome), go to the drome, and the Looks there caused me to unclip - most likely the pedals, but I replaced my cleats after.....no problem at the January session.
Tricky sometimes....... the wear on cleats can be un-reliable sometimes
