Nexus 7 hub chain falling off

I would like to ask.
My wife has an electric Kalkhoff Agattu, with a Nexus 7 gear hub.

I am struggling to adjust the tension on the chain - it keeps falling off, even if the tension seems correct.

I bought the original Alfine chain tensioner but did not notice it was one for vertical dropouts - it will not fit into the bikes horizontal ones.

I can play around with the wheel position but I have stripped one nut already, would rather use a tensioner. Are there any recommendations?

How do you check the tension?
Does the chain come off when pedalling or back pedalling?

If you have horizontal drop outs, these are the best way of tensioning the chain.

You could fit a narrow/wide chain ring.

Look for tug nuts for horizontal drop outs.

These should hold the rear wheel in place.

Edit, I think this is what the previous poster meant by tug nuts?
