One of my old panniers jumped off my bike when it was loaded with a mandolin, of all things. It made a good noise as it hit the road!
My Altura panniers have never detached themselves, they're much more secure.

I have this really rubbish one, which came off and broke my mudguard.

On a similar note, has anyone else managed to get a loose bungee caught in their wheel? That was an interesting and rather scary experience. I was riding along quite fast, when suddenly I felt some resistance, then my back wheel locked, I just managed to control the skid, and then it suddenly started rolling again. The bungee had hooked itself on a spoke and wrapped itself around the gear block several times until it eventually snapped.

I've had similar. The hook on the end of the bungee caught a spoke and the back wheel suddenly locked. Managed to stop without falling over and then when I lifted the back of the bike to see what was wrong. At that point the wheel span as the bungee unwound and then out fired a bent bungee hook missing my leg by an inch or so.

It does bug me when drivers tailgate. The last one, in a white BMW, did drop back to her credit when I stopped pedalling and made a pushing back sign with my right hand palm towards her.

I've lost a saddle pack but not panniers (my own fault for not putting the strap round the seat post as well as clipping it in). Both my panniers and office bag clip firmly to the rack.

Well done, it isn't easy to get a good noise out of a mandolin.

My "panniers" (tailbox fairing) is permanently attached, and lockable, not that it'd stop anyone prepared to break the fibreglass:
