This is why I asked. It seems an unusual occurrence in cycling paradise.

Sweden, there are thousands upon thousands of cyclists in cities and towns didn't stop my 14 year old friend getting run over and killed on her bike!
can happen anywhere be it Korea or Russia or Holland or the UK full of potholes

162 cyclists killed in NL in 2010 (can't find more recent numbers immediately) - I assume a fairly low percentage given the number of cyclists
