Red Lights but I'd say it's close between that and no lights/reflectives or should I say ninja cyclists.

I almost collided with a ninja cyclist once cant say I ever have with one wearing some sort of reflective/light clothing and no lights.

I said red lights...only because where I am there are so few it's unnecessary. Doesn't really bother me much though.

The ones who really get up my nose aren't on there though. They're the twits who go on about how environmentally friendly and wonderful they are.
