A phone holder that doesn't fall off...

I'm looking for a phone handlebar holder, the waterproof kind that encloses the phone, which can be tightened sufficiently not to fall off. My present device has a plastic nut-and-bolt mechanism that responds to bumps by loosening no matter how well I tighten it. Any recommendations?

I found the quad lock but had the attachment that was superglued to my own case. I got one of the battery cases so it could charge the phone on the fly. Proper super glue the quad lock attachment to the back. All felt super solid. Phone itself was waterproof (enough)

QUADLOCK Bullet proof, you can get an optional ‘poncho’ to make the phone waterproof. I have one on all my bikes and my motorcycle.
