In particular, broadsheet newspapers.

Does anyone know why they invariably have that single sheet somewhere in the middle which is designed to do nothing but fall out and irritate people?

Surely an odd number of pages is no accident day in day out?
 
One sheet does plenty?

Yep - advertisign revenue dictates the pagination of a paper. Tabloids can only go up in multiples of 4, however, a broadsheet printing press has the ability to print a single slip sheet. This, at times, can bring a paper in on budget.
