I use www.bikehike.co.uk or www.bikeroutetoaster.com .
Bikehike uses open street maps (if you change to it in the options), google maps or O/S maps (UK only)
Bike route toaster used google maps or OSM.
Both allow you various export options to file or to gps direct.
Offline on laptop for example - I use http://www.velomap.org/ maps via mapsource. you can download the maps individually free of charge or all of Europe (which is now around the 4Gb mark) for a donation. They don't come with contours built, but you can download them along with some additional software and add them in. We found this option the best for touring in Europe and keeping a log of where we had been (we were on the road for 12 months so rather easy to forget if it was not recorded).
However, for my iPhone I use an app (complete with offline maps) called NavFree which is fairly accurate. occasionally a road is marked as a dead end or not connecting to another when it does, but on a bike that is usually not an issue and I can't see it would be an issue on LEJOG http://www.navmii.com/GPSNavigation/Maps you can buy the upgraded version, but so far I have had no big issues with the free version.
