Cantilever Brakes to Disc Brakes

I have a titanium frame at the moment with cantilever brakes would it be possible to put on disc brakes if so how. I have a Rohloff hub with disc brakes which uses a chain tensioner. would that fit as well thanks for your help.

No I don't think I do is there any way round it i.e new forks with mounts ok. Frame?

New forks are easy (but expensive) enough but I don't think there's much choice on the rear.
