Chain hop

This is sometihng I have experienced before but never thought to ask.... When in a particular gear, my chain occasionally 'hops' over the currently selected cog. Is this because of a worn cog, chain, other?

Assuming solution is replacement of rear cassette,chain, other?

cog could be worn down, would have to have taken a fair amount of abuse to do so. how old is the bike?

I've got one that does this trick - cassette fine, probably 1700 miles, chain's done 600 ish, it works fine on all gears in outer & middle, but granny ring 2nd jumps like a frog in a frying pan. I suspect the rear mech top jockey roller's shot. The bottom one was just a ring of plastic when I changed it at Xmas, the top one's probably got no teeth left. We'll see tomorrow if it's not snowing.
