'Chipping' a cat

Long story cut short. We have a kitten. About 6 months old. Went for 'snip and chip' the other day. A couple of hours after he was back from the vet, we noticed a small piece of white plastic hanging from his skin between the shoulder blades. It was the chip. About an hour later it fell off. Back to the vet for a new chip last week ("it happens occasionally" said the vet). I checked Gary (yes, that's his name) when he came back and could definitely feel the chip under skin (it's about the size of a grain of rice), but sure enough two or three days later the chip was discovered on carpet. So, is the cat getting it out himself (it's between the shoulder blades - where they're not supposed to be able to reach), is his body rejecting it in some way, or is it poor work by the vet?

On topic, we had ours chipped when we got him and as far as I know, it's still in there - he was about two when we got him. My cat is named Oscar, but I've always wanted a dog called Gary - long story.
 
Years ago we had two cats chipped, and one just popped out after a few days. It was in her leg. She definitely didn't fuss with it, we noticed it sticking out and removed it.

(We didn't take her back to get rechipped, as these were rescue kittens that were fixed by the charity as part of the original adoption price - after one cat not being properly chipped and previously another had died under anaesthetic, we weren't taking them back there. Which is how we ended up with cats named Guildenstern and Laertes)

Our cat is chipped.... he now has a cat flap that reads his chip so that it only opens for him... it's brilliant!

Off topic; where did you get it from and how much please?

We're looking at a cat flap, if only to save one of them waking me at 5.30am and the other leaving puddles, but haven't found one. And given the number of cats in the area a chip-reading cat flap seems sensible.
