Hand pump / CO2 cartridges to get those spare tubes inflated
Towel, shampoo & shower gel
Eyewear (especially if you wear contact lenses)
Lock & key
All the regular stuff you have on you at the target location, like your office security pass, underwear, socks & office/work outfit (including accessories that are easy to forget, like cufflinks), plus your home keys.

Deodorant and comb. Enables you to cycle into work, but still be able to charm the ladies in the office, avoiding helmet hair and stinky armpits.
Apparently. I'm an engineer, and therefore work in an all-male office. And get home to a wife who is contractually obliged to give me a hug regardless of my condition. (But I do have a shower the instant I get in.)

Forget all the above - just duck tape and WD40. All life's problems are caused by things coming unstuck or breaking when they shouldn't, or things not moving when they should. Select one of the two above items depending on which problem you are facing.

I was amused by a fellow URC'er's tale of fetching a friend from the train station. He took his Burrows 8-Freight with a brompton in the back. They then rode back with her on the brompton, and her luggage in the 8 Freight. I guess that counts as a spare bike!!
 
Swiss Army Knife (everyone should have one)
A few zip ties
Small first aid kit - gloves, couple of dressings, resus mask, triangular bandage (everyone should have First Aid training)

One of those reflective survival blankets that people get after marathons. Seriously, one would be handy if you see someone who's been knocked off the bike, or if you breakdown in bad weather.
