I have filled it in but I was a little confused how to answer how much it cost for basic maintenance since I would do the basic tinkering with the things listed myself - but use the bike shop for other things or more complex problems with those bits. It might have been worth recording what type of environment we live in - I'm near a city so plenty of bike shops and even more than one mobile unit to choose from if I needed to.

Not a good one, maintenance costs relate to the work being done, not some notional figure. If I was to have a headset and bottom bracket replaced, I would expect to pay more than simply having a new chain fitted.
