Some of the questions which don't have a "none of the above" option certainly need one. For instance, I tend to pick up my cycling tops on sale racks at triathlons, occasionally in Aldi, or from my club. The zip style varies depending on whether I am looking at a warm long-sleeve winter jersey or a cool short sleeve summer top. I'm afraid I didn't complete the questionnaire because of stuff like this.
