and ebay looks like the best place to source a replacement, is the size measured on the internal or external diameter?

external dia , also check for 27" or 700c wheel size .

Most (but not all) old steel bikes will have a 7/8" outside diameter quill stem (easy to measure without taking anything apart) and will therefore require a 1" threaded steerer.
But you need to get the right length of steerer.... or at least one which is threaded far enought down and then cut off the excess length.

I bought a set for a Carlton about 2 years back, brand new but chromed. If I remember where from I'll add it to this post. I take it it was threaded??
