Ran across (not literally) two today with the slots parallel to the pavement. I assume they have been incorrectly orientated? Is this common? They could be lethal to roadies. I will nip out tomorrow to log them and report them.

It is common. Avoid at all costs. That is usually how they are designed and they should only pivot open one side.
 
learnt that lesson the hard way as a child back in the '80s. One road in particular was seriously bad for parallel slot drain covers. Front wheel did find the slot and I did go over the handlebars... luckily it was my mother's bike not mine!
Don't see as many of them as I used to thankfully, but the latest theme seems to be badly placed drains, where the road has been widened but adding a 1 foot wide strip to the side of the road and the grids left where they were usually on bends!
