Ive never fitted, used or even seen one till one arrived today in the post. But what a simple but very effective answer to dropping the front mech down when going to a sub compact chain set from a compact chain set when you have a brazed on front mech bracket.

Would the altered position of the mech in the horizontal might be an issue for some bikes?

I dont think so, because by fitting it and dropping the front mech down, is only what you would do if it was a band on bracket. That said, they also make a front mech extender that is machined at 4 degrees, but no idea what application that would be for?
 
If you start with a road bike derailleur designed for a compact double.
Then the angle of the adapter rotates the bottom of the cage down.
That way it's slightly closer to the small chainring of a sub-compact.
So you can still use your old derailleur with a sub-compact and still get crisp shifting.
