Wore my third string cycling shoes this morning: an old but good pair of Sidi 'Terminators'. The model where the top strap tightens with a lever and ratcheting mechanism.
Got home and tried to take shoe off, but the ratcheting mechanism had jammed and the strap would not loosen. Shoe was on so tight that nomatter how much I wrestled and cursed my foot would not come out.
Not being a contortionist, I was not able to get my foot close enough to see what was going on, I had to resort to cutting the plastic strap with a stanley knife.
Examining the shoe afterwards, I was able to see that a small spring had worked loose, this not allowing the ratchet to release.
