I normally carry my wallet with me wherever I go, so there's some ID in there. However I do like the idea of an ID tag, and had though about getting something similar to dog-tags before...

I am known to wear a dog tag with name, address, phone number and blood group on it. I've also got my mobile with me with contact numbers on it. Then if I've got my purse with me, there's my British Cycling membership card in it, so ID on that too.

I have the Road ID and try to wear it on any ride or run partic when alone. They are pretty cheap and I think amazing value. And it arrived within a pretty short time

I have a roadid from the link given. It was about the same price including shipping as getting one in the UK (can't remember the addresses for those - something like'ice id'). Plus, it came in purple, whereas the UK ones I found didn't! Yes, this was a very important factor

I do have ID on me - when commuting, my driving licence is in my wallet which is in my handbag which is in my pannier. However, I like having the ID round my wrist - first responders generally look at the wrist or neck for a medic alert or similar thing, so they would find my ID pretty early on. It has my name, DOB, and phone numbers for home, husband and mother.
Besides, if I were in some sort of incident where I was incapable of telling them who I was, who knows where my pannier/bike/etc would end up. And, they're not going to take the time to go rooting through my things until later on.
So as I see it, having the ID on my person rather than in my luggage means my husband will get the bad news that much quicker

Having said all that, I'll happily admit to being an anxious person (I have a diagnosis to prove it and everything ) - so I'm generally a little more paranoid about these things than most people.

I have the roadID, a black one with my blood group and contact number on it. The service was a little slow receiving it from the states. It took about 3-4 weeks to arrive.

There was a national campaign sometime ago that recommended everyone put "ICE" before a contact on your phone. The emergency services know who to contact In Case of Emergency.

I printed out a simple ID with all my details on and laminated it and wear it round my neck. On the back I also printed off a checklist for all the things that I do prior too a ride and also take with me on a ride. Doesn't work though as I still forget things, monday I forgot my pump.

Putting your blood type anywhere is pointless. The first thing you'll get, if you need blood, is O neg. Once that's underway you'll be tested and given type-specific blood. No paramedic/doctor/etc. is going to trust anything printed on a bit of paper or on a dog-tag. Get it wrong and you die. They'll find out for themselves.

That's what I was informed when I asked my doctor once what blood type I was (still don't know). She said it really wasn't necessary to know.

I have some dog tags that my wife makes terrible fun of.
