I have only ever removed the screw on type cassette and only needed the adaptor to fit the splines. I then just held on the the rim and turned the spanner and adaptor. Don't know if this helps your problem.
You can make your own chainwhip, but as you can buy them for six quid why bother?http://superstar.tibolts.co.uk/product_info.php?cPath=36&products_id=388 You'll use it enough times in a bike's lifetime, and thought here are other ways of holding the cassette in place, a whip saves skin and knuckles. Similarly a lockring remover costs from a fiver, and there isn't really a way round them that I know of.
