Take up a bit of slack in the cable, have you checked that when you are in the gear you have selected say the middle one that the chain and mech are nicely in line with that sprocket.

Yeah, could just be cable stretch. Adjust the rear gear cable slightly, see if you can iron it out.
 
I would have said your cable tension may need adjustment, this is really easy to do and involves putting the bike in the smallest cog and fine tuning the tensioner until the gear change is exact.
