Commercial stuff in general. My last employer is on W7, having moved on from XP about 3 years ago.
I don't suppose many businesses that are running Vista or W8

They aren't networked to the outside world so could happily run XP indefinitely.
Iirc NT4 had some kind of military certification as long as it wasn't connected to a network. (irony is my point).

I have a friend, who is now past retirement age, he still works one day a week at Barclays head office.
He is a COBOL programmer

They still have mission critical systems that rely on COBOL, which has been around since 1958!

We have a couple of old laptops in the office that are deliberately not updated because they run old versions of Sage. The everyday PCs run the latest version, and we use these to upgrade backups from clients who use old versions - Sage will only restore backups from a couple of versions ago. One of those runs Vista.

...and the Amiga IIRC.

I can still run PlayStation 1 games in my playstation 1. If I don't need to run anything else, why upgrade? Same goes for Windows XP..
