Rotary wire brush (flat, not a 'cup brush' type) in a drill or angle grinder works wonders for prep. work then fill in the bare bits before painting all over (2nd coat over those, 1st on main area)+1 for smoothrite. I have a 5 litre tin of it! I have done the radiators in my house (old cast iron) my windows (also old cast iron) and my front gate (you guessed it).
I scrape off the rusty bits and lose paint but then paint over whatever is left. I rather like the look of 6 coats of paint on old things.
I know what you mean about that 'distressed' look.
