It's a survey relating to bikes and helmets and you apparently wear one, don't wear one or not applicable? Couldn't say that I sometimes wear one.

Agree with summerdays I'm an occasional user and not necessarily for the reasons other people wear helmets. I hijacked the subsequent question's comments box to answer that question accurately.

I'm confused by question 8 too: have you had a bicycle or helmet stolen. Why join them together? They are totally separate entities. I suspect George locks his helmet to his bike when he leaves it or assumes everyone does. Else that or is seeking to skew the results in some way to suggest helmet theft is as prevalent a crime as bike theft ????

Same here. In the main I wear them but in hot weather I don't.
