There is probably a 30mm diameter solid steel drive shaft with some big bronze bevel gears in the BB.

I wonder if it's a Paralympic blind cycling bike? Where the athlete at the back is there to do the work and the sighted guide at the front is just there to steer?

Thus the pedals to stand on but no ability to help the person who is actually competing.

It would handle like a tank with that wheelbase. Probably quite fast in a straight line, though.
