When people say put your repair kit in a bag on the bike, they don't mean a big pannier, but a little bag under the saddle like this http://www.wiggle.co.uk/saddle-bags/ - sort by price and you'll see some of the little ones. I have an inner tube, multi tool, chain tool, chain link, tyre levers and a piece of string in mine, and the pump attaches to the frame. Fit and forget (but don't forget to replace the punctured tyre when you've shoved it in your saddle bag rather than leaving it around the place....)

Park PRK (self adhesive patches) is about 1" square and 1/8" thick, can often buy with a set of Park tyre levers (that you need anyway) so might as well carry that in case you should run out of spare tubes.

latex disposable glovers
tiny bottle of chain oil (free sample from SJS)
spare gear and brake cable
cable ties
that pouch will just fit a jersey pocket although typically lives in a Topeak wedgebag along with a smaller pouch as a first aif kit
wound dressings, roller bandage, elastoplast, nurofen, small roll of insulating tape - tearable so no need for scissors, can double up as strapping for body or bike.

I do own a CO2 airchuck & cylinders but rarly carry them. Lezyne road drive (on frame at all times) works out lighter than 3 cylinders which is the minimum I'd be happy with on a long ride.

Small saddle bag, tube, multi-tool thingy, crank extractor, repair kit, 5 quid, decent pump. Can pretty much get bike into a position that I can at least get home or to the nearest village. You don't really need to carry much.

Also leave it on the bike all the time.

All in Topeak Aero Wedge under saddle bag, medium, - 2 tubes, 2 CO2 cartridges & injector, Rocket Micro CB pump, 2 Bontrager tyre levers, VAR tyre levers, tiny multi tool, 8mm allen key (for my pedals), double ended flat/cross head screwdriver bit, puncture kit, Park patches, quicklink, Park chain tool, 2 AAA batteries, spare handkerchief, pair disposable gloves. I also leave it on the bike all the time.
Cash ID & cards etc in jersey pocket, rain jacket, gilet, thin wind jacket (as required) in another jersey pocket.
Phone, gel/energy bar/flapjack, keys, whatever, in a Topeak Tri Bag behind the stem. I try to avoid carrying anything hard/sharp actually on me - just in case.

Thanks to everyone, sounds like my little backpack might not cut it after all! its good tho, sounds like I do need to think about it more.

Kinda worried that some of you seem to go out packed up like its a warzone out there! "...and in my left pannier I carry an Uzi 9mm"
