It's the front derailleur, because it's at the front of the drive chain system. The rear derailleur is at the rear of the drive train, and coincidentally, at the rear wheel...

The front changer is definately in the wrong position and one of the teeth on the small chain ring looks worn. Check the shape of all the teeth on the chain ring and the rear casette. They should have a short slightly inclined flat top. If they are pointed they are worn and will cause the chain to jump off.
