what she needed including a 4Gb Toshiba Micro SD card sold by Amazon (not fulfilled by Amazon).

Amazon was the only company involved

If Amazon didn't fulfill the order then there must have been a third party but you contradict this by saying only Amazon was involved. Who fulfilled the order?

Exactly, I've had the odd issue with items bought on amazon, where over the last few years, I've spent £x000 on anything from a cheap SD card to High end cameras as well as clothing, cycle parts and household goods. Looking at their 'fail' rate they are rather good, although last year their next day delivery had a bad patch of several months where stuff didn't turn up as promised, this seems to be over and they're back to a good service.

Their customer services / refunds are first class.

IIRC a couple of couriers had issues, one went bust and another was overwhelmed and stopped delivering

Yes they were/did. However my problems were caused (According to Amazon.) by failures in the warehouse. Twice this was because they handed the package over to the wrong courier, so it was marked up for, say, DPD and handed to Royal Mail. Another three were down to the wrong item being sent. A couple more were simply not despatched on the day of ordering, despite being ordered in good time. All of this before the failure of one company at Xmas last year. By September last year the service seems to have returned to its normal efficiency.

My understanbding is that the OP is saying the order was with Amazon themselves and didn't involve any affiliates - i.e. it was not part of the "fulfilled by Amazon" scheme

A OK, two ways to read the same sentence. Don't you love the English language lol.

This is weird. 4GB cards are practically free? Who would substitute 256mb cards? Anyone is going to notice as you'd fill it in seconds, and the returns will bankrupt you. You might get away with substituting 4GB for 128GB, but even then I doubt it.

I'd put this down to some weird error, rather than criminality.

Amazon sell there own goods and do Amazon fulfillment where the deliver other people's goods. I'm saying it was Amazon who sold and delivered, they didn't just deliver, i.e. it was not a "fulfilled by Amazon" order
