As a van driver I can say that sometimes you just don't see cyclists . I have had to slam the brakes on a few times because a cyclist is suddenly in front of me and I have no idea where the hell they came from .
I was talking to another van driver about this who also cycles and he thinks its something to do with the brain seeing a bike as a slow moving object and blocking it out . No idea if there is any truth in that but its possible .
Also as said above the pillar or wing mirror can obscure the view .
