My method:
1. All incoming work to top of pile.
2. When asked what's happened to 'X' remove it from the pile and, if it's important/crucial process it. If it's not put it back but at the top of the pile.
3. Every month remove bottom third of pile and bin it.
One of my managers used to split the stuff in his inbox into four piles on his desk every morning.
Urgent/Important
Urgent/Not Important
Not Urgent/Important
Not Urgent/Not Important - This pile went straight into the bin.
He was veery keen on having good time management.
