I've never worn padding and I've always worn underwear.

In normal circumstances just the shorts, but I have several pairs of 'balding' shorts which would be totally obscene worn solo but which are otherwise perfectly ok so I wear 'deflector shields' under them - very lightweight seamless black lycra pants.

It was an experiment which was surprisingly successful so in future I will not just bin shorts as soon as they start to wear out.

I've worn thermal LJs under cycling padded bib longs in cold weather and have suffered no ill effects. However, if you start getting a bit sweaty, it turns into torture as the seams on the LJs dig into your flesh where your bum meets the top of your thigh. They just saw into your skin. Not nice at all.

Commando if riding for more than about 15 minutes. That said for 15 minutes, I might not bother with cycling shorts. There are times of the month when I might wear undies under Lycra

Oh and ladies. Tight black leggings over knickers is not good. You show the colour of your kecks to everyone. Buy proper cycling bibs that have thicker lycra please.
Sadly this doesn't just apply to cyclists. Leggings are not trousers, if they were theyd be called trousers!
I do wear leggings under my skirts or dresses, preserves my modesty when cycling.
As for the original question I wear knickers under my padded pants, never found it uncomfortable

Compression shorts under bibs in the summer and old see through bibs under tights in the winter , used cotton undershorts once but they got saturated in sweat and made the ride very unpleasant .
