One of the last cyclists hit the marshal. The car leading the pack stopped just in time and went to give aid. What happens after is just shocking.

The initial shouts of 'left' would have confused the lead riders & no doubt caused a ripple effect through the bunch hence the rather shocking secondary incident.

Yup, the marshals shout left, which is correct, from their perspective, but is actually right from the racers perspective!

From reading the comments of the video the bunch was following the car?!

Happens a lot in criteriums. A rider falls and is flat out on the circuit, and when the pack comes round, there is a mess with riders hitting marshals, parameds and the barrier, let alone hitting other riders.
When one is in a race, one cannot hear any shouts from up ahead, and one is not necessarily looking too far ahead, usually at the road maybe 20 yards, until its too late.

A lead car is common, but I am not sure if this was indeed a lead car in this instance.
