my wife was delivered on the bathroom floor by the neighbour...

i think it happened a lot before mobile phones. And home phones for that matter...

A midwife? Not many of them outside of the NHS any longer: even if it weren't the case that their professional body are playing silly buggers with insurance requirements, the costs go into the thousands (albeit that is for prenatal and postnatal care, not just delivery alone). Or were you thinking more of a doula?
