The interesting thing to me is that the poster puts all the responsibility on the person in the car to look, not the cyclist to avoid... would a 21st century equivalent show the cyclist riding wide with a warning about the Dangers of Doors?
If that poster is the only evidence, I'd say the paparazzo on the bike was trying to take an upskirt of the driver as he was getting in and just fell off.

Thats the first thing I noticed when I looked at the poster, no advice to the cyclist to avoid the door zone, just advice to the driver to take care, which is how it should be.
