Could be a manufacturing defect and you were unlucky. However are you sure it was seated properly in the guide slot? May have sheered after repeated flexing due to catching in the mechanism.

In 45 years of bike fettling I've never seen one like that!

I had something similar on my Carlton but I put it down to incorrect installation as the handles I have have a piece which is supposed to turn as the brake lever is pulled but it was stuck on mine.

I bought some cheap cables from Wilko's as i was fixing a bike for someone (for free btw)
When i fitted and was adjusting the brake, the nipple snapped off, just like your pic. I put it down to cheap parts tbh

I've never seen it with quality cables
