Hi everyone
I want to give the bearings a winter clean grease up in the hubs but I can only get to one side of these putting a hex either side will release one side and I have got to the bearing but I'm at a loss how the other side comes out as they both spin same way

Your pictures are terrible. What make of wheel/hub is it. What sort of bearing - cartridge or cup and cone with loose bearings.

I suspect only one end cap is coming off as that's the looser one, and once off it will just spin. Is there any way of holding the axel or inserting another 'larger' hex key into the end where the cap comes off.

It must come off as otherwise you'd not be able to change the bearing. You can tap an axel out but with the end cap still being on, you'd still struggle to change or access the bearing on that side as that will come out too.

You've got to grip the axel to get the other end cap off - it's in the video, so if you don't want the bearing/axel out you need to find a way of gripping it.

That's how you get bearings out on some wheels - if you can't grab the axel, then it will need to come out to get to the bearing.
