Is it an Unwritten Rule in the HW Code; That on a two lane approach to a rbt and a cyclist is circulating that the car in the outside lane must give way and the car in the inside lane must not ? It seems to happen a lot

Probably because the inside car's view is obscured by the outside car. The one that sees you stops. The one that doesn't - doesn't.

It must be too complicated to figure out the cars on their right are stopped for a reason

The drivers in the RH lane seem to get very upset about bikes in front of them taking the lane, and the ones in the LH lane are just too (choose one) - ignorant/ impatient/ stupid/ anything else you can think of - to do as they're supposed to and give way.
Roundabouts are something that in a world full of sane people would be fine. The world isn't and they're not. That applies IMO as much to driving as to riding.
