my bike fitting determined where my cleats are positioned, not sure how else you could work it out other than by trail and error

Im not the best one to ask as i am still getting a new pair of shoes to work but i read once you have got your bike fit right then the pedal axle needs to be about where the balls of the feet are , then the angle depends on how your feet are naturally when walking so your not twisting your knees.

Depends on the type too - less important side to side as you have the float wih some systems.

I read all the online advice in setting mine up initially but after getting repeated pain on the outside of my knees during longer rides I invested in a pro-bike fit. While my cleat position was okay in terms of its position along the length of the shoe, they moved the cleats right over to one side and inserted some wedges between the cleat and the shoe. I would never have arrived at the position they set them at on my own and I haven't experienced any knee probs since the bike fitting. Not that helpful, but I would echo the above in recommending a proper bike fitting as time and money well spent.
