Red rear reflector came off?

Those pedals come with the sissy cleat (as MTBers call it) - are you using those? They do release much more easily than the standard black SM--SH51 cleats.

I use A520s on several bikes. I think (although I haven't measured it) they give slightly more ground clearance than double-sided SPDs, which is useful on a fixie.
 
Cant say i have reflectors on any of my bikes, then again i am currently running 4 rear lights

Which are, yes..... reflective. both front & rear
It's the first section, that vehicle headlights strike, & the movement of the rear catches light too (also sunlight, if it's behind you)

And, those 'Snappies' (Slappies?) on wrists & ankles, when its busy
Ie; if I'm leaving at 20:00, the same time as most of the visitors

When the weather gets colder (far too warm still), the Boardman '360' jacket will come out
To all intents & purposes, it's made entirely from the same material as the stripes on a Hi-Viz jacket

Why All That?
Mainly because it'd be too embarrassing, to go back to work;
- In a big yellow truck (ambulance) with Paramedics I probably know
- have clothes cut off/be treated by A&E Nurses/Doctors that I know
- be pushed about by Porters that I know
- mickey taken by Radiographers

And, possibly be spoken with by a Traffic Officer that I know, too

Yep "Far too warm" especially for those boil in the bag type jackets.

Great night time visibility.

You're not going to be stopped by the police for lacking reflectors if you have a decent rear light. However, it gives a defence lawyer an excuse to reduce compensation if you get rear-ended. After all, your bike wasn't roadworthy.

I think the red rear reflector probably dates from the days of *no* mandatory rear lights, and remained useful with dynamo lights before standlights became almost universal. It's of marginal use with a typically bright battery LED.

Pedal reflectors are a bigger problem because they are problematic with clipless pedals. You can get big reflector attachments for double-sided SPDs which reduce your pedals to single-sided SPDs. That's hopeless.
my favourite ever pedals were a set of look deltas which had a reflector front and rear...
