Who is responsible for it... there has to be someone to monitor its use,

Suggest that the vehicle has been left in an unacceptable state and that the company should clarify that the responsibility lies with the borrower to return it in an acceptable manner.....perhaps for a short period check it in and out to ensure compliance

I would also suggest reporting it to the PA and also tell him/her the 'polite' reply you got from her when you mentioned the smoke problem!!!!!!!!!!!!!!!!!!!!!!!!!!!!!
