My understanding is that 1 1/8 inch steerers are all threadless. Am I wrong, vintage gurus?

You are - my DB is threaded 1 1/8.

Kona MTBs used 1 1/8" threaded steerers for years (at least 1991-94), claiming quill stems were where it was at. Brompton still use them. The Kona ones have quite a low stack height but a Tange Falcon, or whatever the 1 1/8" equivalent of the Falcon is, is a good replacement.

1 1/8" was originally Tioga "Avenger" size, in the same way that 1 1/4" was Fisher "Evolution" and threadless was Dia-Compe "Aheadset" (Cane Creek now own that particular trademark).

I had a 1990s kona recently and the threaded headset was a thing of beauty! The locknut had a clamp bolt built in which totally removed the one weakness of a threaded headset in an mtb application, namely coming undone due to the continual impacts of riding off-road.
