Eyes. Even when going just to the end of the road and back, to check everything is ok on the bike, and I'm wearing non-cycling specific apparel, I'll always put glasses on, as my eyes stream at anything over 5mph.

I wear a cap more often as the hair gets thinner. The peak keeps a lot of stuff out of the eyes as well. I pretty much always wear mits.

I protect all the usual stuff, plus the area round my left hand shoulder. The outline of my pacemaker and the leads is already pushing out on the skin and It would hurt like mad if that took a bang.

Constant apparel for me are gloves for protection and for dulling road vibrations, a buff around my neck even in summer and a cap or woolly hat.
I suffer from tonsillitis and head colds, if there's any slight breeze I get sick if riding uncovered.
