It's a joke (quality of the joke set aside for the moment) about a bloke who thinks he's being cheated on. The story told by an imagined first hand narrator. Punchline about a car fault being more important then an imagined and allegged affair.
Why the angst?
The reason (I think) people are getting upset is because the joke seems to trivialize something (adultery) that can destroy families and wreck peoples lives.
Easier to understand if a cheating partner is something you have had to deal with.
