It's a Microsoft office form. Is there any way to confirm that it's genuine?

That is a generic privacy notice from over 7 years ago. I could put a link to it on my own personal website and it would be useless/meaningless as it's a link on my site. Anybody on any website could include that link.

And that it's over 7 years only would raise questions in my mind.

How does that confirm it's genuine? Also, it can still record your IP address, some cookies and browser fingerprint, which may be personal information in some cases.

It doesn't confirm it is genuine, but it makes it less likely it is a phishing attempt.

it would concern me more theyve taken 4years to even begin to ask questions like this, unless its just a study for Leeds university to publish a research paper on for me, which will no doubt uncover the shocking revelation that some people find cycling difficult when cars travel too fast and too closely on badly maintained roads.

equally I dont think its a particularly well thought out survey some of the questions and the answers you could give didnt seem like they were structured properly.
