R.I.P. :-) There's too little to go on but it seems like every fatality coming out of the London cycling community follows the same pattern, surely some sort of mass action could be implemented?

nasty junction, only really one motor lane wide, but cars try and squeeze it into two leaving no room for anyone on a bike.
there should be zero deaths like this inside central London… average traffic speed is so slow this could be designed out totally.

We've been 'standing' for quite some time now. It doesn't help that TFL are very motor vehicle centric.

Maybe a better voice, or a better way to raise awareness is the problem. What is the LCC doing about it, the CTC? Get it in the newspapers, bring central London to a standstill during rush hour. Critical mass on a Friday night, who really gives a shoot about that?

We're you there for the Tour de Danger last month. 300 cyclists were. If not why not?
 
Could some cyclists also be dangerous?

In some cases maybe but the evidence is that in the overwhelming majority of collisions the police place the blame on the driver. In the last three deaths all three lorry drivers have been arrested.
